Payment Instructions

When registering online for the CMHDRE's 11th Health Disparities Conference, you have the opportunity to pay by Credit Card or Institution Purchase Order/Check.

NOTE: If payment is not received by the respective discounted deadline, then your balance due will be adjusted according to the timeline and respective rates as noted above.

ON-SITE PAYMENTS: Please note that neither Purchase Orders nor Personal Checks will be accepted on-site. Attendees must provide a valid Credit Card or a Business/Institution Check at time of Check-in to cover all Registration Fees. When paying on-site, please note that on-site rates will apply.

Cancellations

Notification of registration cancellations must be submitted in writing to the XULA-COP2018 Organizers. Registration cancellations occurring prior to January 16, 2018 will be assessed a 50% cancellation fee. No refunds will be granted for cancellations on or after January 16, 2018. Refunds will be distributed no later than 30 days after the close of the Conference.

The request for a registration refund must include the tax identification number of the company or institution if registration was paid by a company or institution check.

CMHDRE reserves the right to cancel activities prior to the scheduled date should uncontrollable circumstances make it necessary. Each registrant will be notified by the email address(es) provided during the registration process. In the case of activity cancellation, the liability of the CMHDRE is limited to the registration fee.

Conference Management reserves the right to limit the number of participants in a program and is not responsible for any expenses incurred by an individual whose registration is not confirmed and for whom space is not available.

Special Assistance

All attendees are required to indicate all special dietary or ADA accommodation needs when registering for the conference.
